Chapter One:

Introduction

1.1 Background

Community Health Worker (CHW) programs have gained significant attention and recognition as an essential component of healthcare systems worldwide. These programs utilize trained individuals from local communities to deliver basic healthcare services, health promotion, and disease prevention interventions. CHWs act as a bridge between communities and formal healthcare systems, especially in resource-constrained settings where access to healthcare services may be limited.

Nigeria, as Africa’s most populous country, faces numerous healthcare challenges, including inadequate healthcare infrastructure, a shortage of healthcare professionals, and limited access to quality healthcare services, particularly in rural areas. In response to these challenges, Nigeria has implemented CHW programs across various hospitals to enhance primary healthcare delivery and improve health outcomes for its population.

1.2 Research Aim and Objectives

The aim of this research project is to explore the challenges and opportunities associated with scaling up CHW programs in Nigeria, using a comparative case study approach across multiple hospitals. The objectives of this study are as follows:

  1. To assess the current status of CHW programs in selected hospitals in Nigeria.
  2. To identify the challenges faced by CHWs in delivering healthcare services in different hospital settings.
  3. To explore the opportunities and potential benefits of scaling up CHW programs in Nigeria.
  4. To provide recommendations for effectively scaling up CHW programs based on the findings.

1.3 Research Questions

To achieve the research objectives, the following research questions will guide this study:

  1. What is the current status of CHW programs in the selected hospitals in Nigeria?
  2. What are the key challenges encountered by CHWs in delivering healthcare services in different hospital settings?
  3. What are the opportunities and potential benefits of scaling up CHW programs in Nigeria?
  4. What recommendations can be made to effectively scale up CHW programs based on the findings?

1.4 Significance of the Study

This research project holds significant importance for policymakers, healthcare professionals, and stakeholders involved in the development and implementation of CHW programs in Nigeria. By exploring the challenges and opportunities associated with scaling up CHW programs, this study can contribute to evidence-based decision-making and policy formulation.

The findings of this research can inform strategies for strengthening and expanding CHW programs, leading to improved healthcare access, enhanced health outcomes, and more efficient utilization of healthcare resources. Furthermore, this study may also serve as a reference for other countries or regions facing similar challenges in scaling up their community health worker initiatives.

1.5 Scope and Limitations

This research project focuses on a comparative case study of multiple hospitals in Nigeria, exploring the challenges and opportunities for scaling up CHW programs. The selection of hospitals will be based on factors such as geographic location, urban/rural settings, and healthcare infrastructure.

However, it is important to acknowledge that this study has certain limitations. Firstly, the research will be conducted within a specific timeframe and resource constraints. Therefore, the findings may not be generalizable to all hospitals or regions in Nigeria. Secondly, the research will rely on data collected through interviews, surveys, and document analysis, which may be subject to reporting bias or limitations in data availability.

1.6 Organization of the Research

This research project will be structured into five chapters. Chapter One provides an introduction to the research topic, and outlines the research aim, objectives, questions, significance, and scope of the study.

Chapter Two will review relevant literature on CHW programs, scaling up healthcare interventions, and the Nigerian healthcare system. This literature review will provide a theoretical framework for understanding the challenges and opportunities of scaling up CHW programs.

Chapter Three will present the research methodology, including the selection of hospitals, data collection methods, and ethical considerations.
